Subsidized housing for seniors and disabled persons is located throughout the greater Phoenix area in the form of independent apartment complexes. Rent (which includes utilities) is approximately 30% of a person’s adjusted income (medical expenses are deducted). To qualify for housing your income must meet the guidelines at each particular complex to which you are applying. You must also meet the age requirement of 62 years/or (if you are younger) have a disability (usually a physical disability that impairs mobility). To apply, you need to contact the individual apartment complex and ask to fill out an application. This will place you on their waiting list for an available apartment. The lengths of the waiting lists will vary according to each apartment complex. The "estimated wait" provided is only approximate. Clients should call the HUD complex to determine the wait at the time of their application.
The following complexes are owned by the City of Phoenix. For eligibility, the maximum income level is $35,950 for one person and $41,100 for two people (effective 2/08). One application can be completed for all 6 facilities. This application is available at the City of Phoenix Housing Dept. located at 830 E. Information Authorization
I authorize the City of Phoenix Neighborhood Services Department to request and obtain all information, including a credit report, necessary for the processing of my housing rehabilitation application. I understand that all information obtained will be held in confidence and used for no other purpose by the City.
Intent to Occupy
I understand that the purpose of the city’s housing rehabilitation program is to correct health and safety hazards in homes owned and occupied by residents with low to moderate incomes. It is not for the purpose of repairing a home the owner plans to sell after the repair work has been completed. I do not have plans to sell the home for which I am applying for repairs.
If I am applying for a loan, I understand that a no – or low – interested rate loan will be made available to me for the repairs costs. I understand that this loan does not require a monthly payment, but that a mortgage will be filed on the property as security for the loan. I will be required to purchase fire insurance with the City of Phoenix names as the loss payee. If my property is located in a flood zone, flood insurance will be required as well. If I am required to purchase fire or flood insurance, I understand that City staff will discuss the requirements with me and will contact my insurance agent.

This authorizes the city of Phoenix Neighborhood Services Department or its designee to inspect or obtain copies of energy use billings from your company for the dwelling at ____________________________ covering a one-year period before Weatherization work and one year after. The data will be used to evaluate cost-savings resulting from the work. Information gathered through this’d authorization will not be used or publicly released in any manner that will reveal the location of the dwelling or identity of the occupants.
Confirmation of Receipt of Lead Pamphlet
I have received a copy of the pamphlet " Protect Your Family from Lead In Your Home", informing me of the potential risk of the lead hazard exposure from renovation activity that may be performed in my dwelling unit. I received this pamphlet before the work began.
